We schedule most initial appointments for one hour and most follow-up appointments for twenty minutes. The initial evaluation is $240 and follow-up appointments cost $110. We may also charge a $50.00 administrative fee for tasks including GeneSight workup, writing letters and FMLA paperwork. Please note we do NOT complete disability assessments.

POLICIES

Cancellation policy: Appointments must be cancelled at least 24 hours before a scheduled appointment, and must be done by notifying the provider by email or Spruce, (not confirming an appointment reminder does not cancel the appointment). Patients will be responsible for the full cost of any missed appointments.

Controlled substance policy: Use of controlled substances is closely considered and strictly monitored. We will not necessarily continue controlled substances prescribed by past providers. Please be aware we are able to give only a limited number of refills. Lost or misplaced prescriptions cannot be refilled. Please also be aware that we do not diagnose Adult ADHD, but in limited cases may treat ADHD with evidence of a formal diagnosis. We work closely with Dr. Robert de la Torre who can provide psychological testing.

Medication refills: Refills are normally given at the time of your appointment. In certain circumstances you may contact us to refill a prescription for a non-controlled substance. Please leave a very precise message on our secure line, including your name, date of birth, exact name of medication (include XR if appropriate), medication strength (number of mg), how many times you take the medication daily and your pharmacy phone number. Please also leave your own phone number in case we have any questions.

Medication changes: Any consideration of medication changes must be done during the follow up visit, where we can fully appreciate your current needs. We will work with you to schedule a return appointment in a timely manner where we can can communicate directly and make appropriate decisions together.
